After School Program Monthly Registration

BJAMS ASP Monthly Registration

This form must be filled out prior to the start of the month in order for your child to attend ASP. If this form is not filled out, the Drop In rate will apply.

  • Students must be registered for the After School Program in advance in order for us to provide accurate staffing and space for each student. Capacity is limited to students based upon state regulations and staffing. If we are unable to accommodate your student on any given day, you will receive an email from Michelle Tomlinson, ASP Director. Feel free to direct questions to Michelle at mtomlinson@bjams.org.

  • *Each student in a family must be registered separately.
  • Please be advised these are the days that you will be billed. Should your child attend on a date not provided you will be charged the drop-in rate.
